Frequently Asked Questions

What are the differences between Lymph Drainage Therapy, developed by French physician Bruno Chikly (the method taught through The UI Institute), and other manual lymph drainage techniques?

Lymph Drainage Therapy (LDT) is an original method of lymph drainage that incorporates Dr. Chikly's years of practice in general medicine and osteopathy. Its concept is based on traditional procedures as conceived by osteopath F.P. Millard (Toronto, 1922) and Emil Vodder (Europe, 1936).

LDT combines precise anatomical and physiological knowledge with techniques of direct listening that enable practitioners to very effectively stimulate the lymphatic and interstitial fluid circulation. In essence, Dr. Chikly has developed a method, consistent with scientific discoveries, that enables practitioners to feel the specific rhythm, direction, depth and quality of the lymph flow over the entire body. From there, with advanced training, practitioners can perform a precise mapping of the lymph vessels -- called Manual Lymphatic Mapping (MLM) -- to assess the specific direction of superficial and deep lymphatic circulation (cutaneous, subcutaneous, subfascial, muscular, visceral, periosteal lymph circulation) and find the most accurate alternate pathway for drainage.

Trained practitioners say sessions are shorter and more efficient; they do not practice a monotonous stroke, but receive very accurate feedback from the lymphatic system; they can recognize when the lymphatic system is responding correctly with their hands; they can quickly find areas of restriction and can assess the impact of their work on the lymph flow at any time.

MLM is a breakthrough in the field of lymphology. The applications of MLM are numerous:

Identification of the specific directions of the lymphatic circulation as well as areas of fluid restrictions and fibrotic tissue
Assessment of a patient's lymphatic condition before and after lymphatic drainage sessions Selection of a specific physical treatment protocol
Preventive application (subclinical lymphedema): evaluation of functional alternate pathways (e.g., in post-surgical mastectomies, assessment of the cephalic pathways, posterior axillo-clavicular pathways, etc.)
Preventive Lymph Drainage Therapy with evacuation toward the most accurate alternate lymphatic/interstitial fluid pathways.

Dr. Chikly explains: "One of the biggest differences [between LDT and other LD methods] is the kinesthetic feedback the practitioners can get from using flat hands. Tuning in to the lymph flow through the different compartments of the body, one can follow the specific lymph rhythm, direction and depth, and activate it. This profound feedback adds another dimension to lymph
drainage, enabling the practitioner to be very precise in regard to the amount of pressure, speed and direction of the hands. This approach can be applied to any tissue with a lymphatic circulation, including cutaneous tissue, muscles, all viscera, etc. We also developed a technique more specific for the interstitial fluid and the venous circulation."

Another LDT process involves connective-tissue defibrillation maneuvers that alleviate fibrotic tissues of many origins (post-surgical, post-radiation, scars, cellulite) and prepare the tissue before the specific LDT approach is applied.

Silent Waves: Theory and Practice of Lymph Drainage Therapy
by Bruno Chikly, M.D.
International Health and Healing, Inc.
Scottsdale, Arizona
(800) 233-5880
www.UI.com
$75; 400+ pages; 2001

For those therapists and physicians interested in lymphology and lymphedema, there are, unfortunately, only a handful of respected texts to turn to for reference and treatment. Bruno Chikly's Silent Waves should be counted as an invaluable text, right along with Waldernar Olszewski's Atlas of the Lymphatics of the Lower Limbs and Horst Weissleder and Christian Schuchhardt's Lymphedema: Diagnosis and Therapy. It is certainly more comprehensive than either Renato Kasseroller's Compendium of Dr. Vodder's Manual Lymph Drainage or the dated Judith Casley-Smith book Modern Treatment for Lymphedema.

As a lymphedema therapist myself, I knew the minute I saw the author of this book that it would be invaluable. Chikly's reputation in the research and treatment of lymphedema is inestimable. He does not disappoint. In fact, he may bring just a little too much to the table.

As hungry as I am for medical knowledge about lymphology, I must admit I skimmed the early chapters about the history of the lymphatic system, who discovered the lymphatic system and the evolution of the system in animals. But after chapter four in part one, Chikly wastes no more time-every chapter deals with pertinent, fascinating and well-illustrated elements of the lymph system: drainage techniques, indications and contraindications for lymph drainage therapy, and manual lymphatic mapping.

Recently published information on manual lymphatic mapping has been muddy and frustrating; the illustrations in this book on human bodies, and the text explaining this intricate technique, are the best I've seen.

For those practicing lymphedema therapy, the book holds few surprises, but this does not detract from its value. It truly is "everything you ever wanted to know about lymphedema or lymphology" in one place. And, it is exquisitely well-researched. The extensive bibliography at the end of each chapter alone makes the book worth owning.

Besides the terrific explanation of manual lymphatic mapping, the other treat in the book appears in part five, where Chikly explains and illustrates uses for lymphatic drainage techniques, other than lymphedema. He gives thorough examples of treatments for pre- and post-surgical applications; chronic venous insufficiency; breast care; cosmetic and esthetic applications; orthopedics and sports medicine; and chronic and subacute infectious diseases, including HIV, rheumatology, neurology and more.

The book's references are a gold mine, and the reprints of many of Chikly's original articles are valuable. The grand scale of uses for manual lymph drainage is enlightening, the style of writing is clear, and the illustrations are some of the best and most easily understood in this complicated therapy. This book is not for all therapists, but if you're studying or practicing in this field, your library is not complete without Silent Waves.
